Business clients of Farmers Insurance, Young Douglas can request a certificate of insurance (COI) here. Use this form when a client, landlord, general contractor, or event organizer asks you to show proof of coverage.

What to have ready

  • Certificate holder name and mailing address (the company or person asking for the certificate)
  • Whether they need to be listed as additional insured, or need a waiver of subrogation
  • Project or job details, such as the job address, contract number, or event name
  • The date you need the certificate by
